Fees & Insurance

Fees

Session cost: $165
Session length: 50-55 minutes

I will keep your credit card securely on file and charge it at the end of each session. I accept all major credit cards.

I have a limited number of sliding scale spots available at any given time for people who would not otherwise be able to participate in counseling. I’m afraid these spots are currently full but I will update this page when a place becomes available.

Insurance

I am not in-network with any insurance companies.

There is a chance your insurance company might cover my services as an out-of-network provider.

If you use out-of-network coverage you must pay my cash-pay rate. Then each month I will provide an itemized bill for services (known as a “superbill”) that you can submit to your insurance company for potential reimbursement.

It is critical to confirm your out-of-network benefits before we begin therapy!

Confirming Out-of-Network Insurance Benefits

*It is important to understand that using out-of-network benefits requires your therapist to provide you with a mental health diagnosis. Let me know if you have questions about this.

Questions to ask your insurance company:

  1. Does my plan include out-of-network mental health benefits?

  2. Can I use my out-of-network benefits to work with a Professional Counselor Associate in the state of Oregon? (Please ensure they cover associates in particular!)

  3. Does my plan cover CPT codes 90834 and 90837?

    • Is prior authorization required?

  4. Does my plan cover telehealth services?

    • Does my provider need to use a specific telehealth platform for me to be reimbursed?

  5. What is my out-of-network deductible? (And what is my policy period?)

  6. Is there a limit to the number of sessions my plan will cover per year, out-of-network?

  7. How much does my plan cover?

    • What is my co-insurance / member cost share?

    • Does my plan cover the full billed charges or only the insurance company’s allowable amount?

  8. Does my plan require pre-authorization for psychotherapy?

  9. How do I submit a claim for reimbursement? Do I need to submit a special form along with the superbill?

  10. Can I get a name and reference number for this call?

Your Right to a Good Faith Estimate

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including mental health counseling services. You can ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
